Forms
The Wisconsin Court System has adopted mandatory standard court forms for some civil, criminal, family, guardianship, juvenile, mental commitment, probate, and small claims cases. In addition, pro se forms for divorce or legal separation are also available. Find circuit court forms.
Some Wisconsin circuit courts have developed forms tailored for local use. Please consult their Web sites or contact the appropriate Clerk of Court . Keep in mind that although some forms are standard for every court, many may differ from court to court in small and not-so-small ways. When you choose to use a form that you found somewhere other than on the court's own site, please ask the Clerk of Court whether the court accepts that form.
