Standard, statewide forms are required by all Wisconsin circuit courts for civil, criminal, family, guardianship, juvenile, mental commitment, probate and small claims cases. More information on mandatory use of court forms is available here. For questions about forms or their content contact the Records Management Committee.
County-specific court forms may be available on a county court's Web site or from a county's Clerk of Circuit Court Office. Try using this site's circuit court links page or the Clerks of Circuit Court directory to contact a specific circuit court.
