CS-225 Instructions

CS-254 Report of observation of court proceedings
To report required court observation. Must be signed by a court official and submitted to the CIP via mail, fax, or e-mail.

Continuing education compliance reporting form: PDF version | Word version
To report a summary of continuing education activities during the two-year compliance period. Must be submitted to the Court Interpreter Program (CIP) by the corresponding deadline.

Continuing education approval request – Participant: PDF version | Word version
For an individual interpreter to request approval from the CIP of a continuing education activity or event that is being sponsored by a provider who is not considered a pre-approved provider . Must be submitted to the CIP at least 30 calendar days before the event.

Continuing education approval request – Provider: PDF version
For a provider of an educational event to request approval from the CIP of a continuing education activity. Must be submitted to the CIP at least 30 calendar days before the event.

Oath of office for court interpreters
This oath of office form must be signed by the interpreter in front of a notary public and submitted to the CIP via mail, fax, or e-mail.

ADA accommodation request form – Written exam
For an interpreter candidate to request an accommodation during the written examination. Must be submitted to the CIP at least 15 business days before the written exam date.

ADA accommodation request form – Oral exam
For an interpreter candidate to request an accommodation during the oral examination. Must be submitted to the CIP at least 30 business days before the oral exam date.

Application for reciprocity – Spoken language
For a spoken language interpreter from another jurisdiction to apply for recognition of his/her credentials and to request inclusion on Wisconsin's Roster of Interpreters.

Application for reciprocity – Sign language
For a sign language interpreter to apply for recognition of his/her credentials and to request inclusion on Wisconsin's Roster of Interpreters.

Record of court interpreting practice and training – Sign language
For a sign language interpreter to report required court interpreting practice and training hours in order to comply with roster requirements.

Grievance forms

Any person may initiate a complaint against an interpreter. However, the CIP’s grievance form is limited to complaints about the following:

  • Violations of the Code of Ethics for Court Interpreters;
  • Conviction of any felony or misdemeanor, in particular, crimes involving moral turpitude, fraud, corruption, dishonesty, misrepresentation, or false statements;
  • False or deceptive advertising after receipt of notification to discontinue;
  • Knowing and willful disclosure of confidential or privileged information obtained while serving in an official capacity;
  • Gross incompetence;
  • Non-compliance with the program’s policies and procedures;
  • Incapacity to perform the duties of a court interpreter, which may include but is not limited to medical incapacity or incarceration;
  • Engaging in an action that questions the interpreter’s ability to work effectively in court.

Note: Any complaints regarding the interpreter’s punctuality should be addressed directly by the county.
